What Is an Annular Tear?

An Annular Tear is a small split in the tough outer ring of an intervertebral disc. The ring is called the annulus fibrosus. A tear may irritate nearby tissues and can allow the disc’s inner material to press outward. Some tears cause pain, while others cause no noticeable symptoms.

Key Takeaways About Annular Tear

  • The tear affects the disc’s outer ring.
  • Symptoms may occur with or without disc bulging.
  • An annular tear is not the same as a herniated disc.

How Annular Tear Works

Annular tear describes damage to the outer ring of a spinal disc. The ring helps contain the softer inner disc material. A tear can change how the disc handles pressure during bending, lifting, or sitting.

Healthcare providers may review symptoms, movement, and imaging when they assess a suspected tear. Imaging can show changes in the disc, but the image does not always explain the person’s pain. A herniated disc occurs when inner material pushes through a weakened or torn outer ring. A tear can exist without that material pushing outward.

When we assess an annular tear during herniated disc treatment, we review the person’s symptoms and movement limits together. We also consider imaging findings in context, because a visible disc change may not match the source or level of pain.

Annular Tear in Practice: A Real-World Example

A person may develop low back pain after repeated lifting, and imaging may show an annular tear in a lumbar disc. The tear may occur with a disc bulge, but the two findings describe different changes.
